What is the Form 1601 ?Printable

Youth soccer players use the Form 1601 to register with CYSA-N (Northern California Youth Soccer Association). The form serves two purposes.




  1. Convey information to CYSA-N. Data like name, address, birth-date, and which team you are playing on.




  2. A document whereby parents or guardians give consent for emergency medical treatment.




Players, Managers, Coaches and Parents can print out a Form 1601 using the data entered into the Player's and Parent's Profile.



IMPORTANT


The 1601 Form on the RedwoodSoccer website is NOT a web based registration process with CYSA.

Using the RedwoodSoccer website is simply a convenient way for you to enter data onto the 1601 Form. RedwoodSoccer only requires limited information about a Player. A Player and their Team can use manually produced Form 1601s to register with CYSA.


Players are not required to use a computer generated 1601 Form. Blank 1601 form


Any other uses for the Form 1601?


The copy of the 1601 Form, containing an original parent.guardian signature should be at every practice, scrimmage and game. The form will be necessary if a player requires medical attention and the parent/guardian are not present.
